IAP Registration Steps IAP – The Indian Association of Physiotherapists Registration Steps Please read the instructions carefully before sending the email for follow ups. Step 1. Go to IAP website http://www.physiotherapyindia.org/forms/applicationforms/index.1.html and chose the appropriate form to download Step 2. Following forms are available. Verify that you are selecting the correct IAP application form. Download the PDF form. (a) IAP Lifetime membership form a. For Non Member College b. For Member College (b) IAP Student membership form a. For Non Member college b. For Member college (c) IAP “Good Standing” certificate Step 3 Complete the form with no fields missing. The details should match with the scanned documents to be submitted along with the form (Details in step 4) Step 4 Please scan the documents and send the following via email to [email protected] 1. Attach the completed form 2. Attach a passport size photograph 3. Attach 10th pass certificate 4. Attach 12th pass certificate 5. Attach B.P.T certificate 6. Attach internship certificate 7. Attach M.P.T certificate [if applicable] 8. Attach any two government I.Ds with photo and address like Aadhar, Voter ID, passport, PAN etc. 9. Soft copy of the signature – Sign on a paper and share a photo/screenshot in the email Any missing documents would lead to delay in issuing the certificate 1 IAP Registration Steps Step 5 While sending the email – please take care of the following for quick Please do not make any payments at this stage. 1. In the subject line – Membership type (Lifetime or student) and your name 2. Attach ALL the above documents in the email 3. Mention the following in the body of the email. The details should match the government IDs submitted a. Complete name b. Complete Address with City, State and PINCODE c. Cell Number 4. Any queries that you might have Step 6 We shall check the documents and confirm in an email, so that you can make the payment Step 7 Payment can be made through following ways.
