ADVANCED FUNDAMENTAL HEALTH COACHING AGREEMENT 1. Payments are taken via credit card or Paypal via Paypal at time of booking. Consultations may be rescheduled at any time, and it is the responsibility of the client to reschedule via the booking link provided in the con rmation email. 2. Appointments may be cancelled prior to the scheduled start time by the client via the con rmation email link provided. Appointments may cancelled with 24 hours notice and fully refunded via. 3. “No Show” appointments (client not arriving to appointment with no explanation) may be rescheduled or refunded only if extenuating circumstances apply, and are subject to consideration. 4. Dave Lee is not a medical doctor, and the information provided does not constitute, or substitute as medical advise, and is for educational purposes only. 5. As a health coach, all supplements, medications, hormones and other diet/lifestyle approaches are discussed on an educational basis only. It is the decision and responsibility of the client to apply adequate personal research, and to discuss any concerns or approaches. with their medical doctor. 6. Advanced Fundamental Health, nor Dave Lee, is responsible for your health. Therefore, as the client, it is your responsibility to take notes, and ensure you fully understand advice before cessation of the consultation. Dave is not responsible or able to provide additional advice outside of consultations, via email or other messaging services. 7. Dave may provide written notes, product names, protocols etc during the appointment. This is to supplement your own records, not to serve as a written report. It is the client’s responsibility to retain the information provided during consultation via note taking. 8. Meetings are not to be recorded. 9. Advisement regarding recommendations made, or future recommendations will not be provided via email or other messaging platforms. Between-consultation communications are to discuss logistical matters only. For further information requests, please purchase “Email Advice option”. 10. Australian clients referred to Performance Health Clinic will become clients of Performance Health Clinic. Dave will act as a consultant for Performance Health Clinic in discussing treatment protocols and making suggestions to the clinic’s doctors for treatment.
