Q1 Informed Consent Title of Study: Public Transit and COVID-19 Follow Up Survey Principal Investigator: Matthew Palm, PhD. This page provides information about the study so you can make an informed decision on whether you wish to participate. Please make sure you have read and understood the entire page. Why is this study being done? To help the cities of Toronto and Vancouver understand how transit riders have changed their travel in response to the COVID-19 outbreak; to identify ways to improve transportation options in Canadian cities if and when the outbreak ends. Do I have to participate? No. Your participation in this study is voluntary. If after completing the survey you decide that you want to withdraw from the study, contact matthew.palm@utoronto.ca. You may withdraw up until December 31 st 2022 at midnight. What is involved with this research study? This survey is designed to take approximately 8 minutes to complete. In the survey, we will ask about your views concerning a variety of topics, such as your travel habits and your attitudes towards public transit and COVID-19. What are the risks of participation? A small number of questions may cover sensitive issues that could create discomfort for some respondents, for example, whether police profiling has negatively impacted your experience with transit. These questions are optional and you can select “prefer not to say” if you do not wish to answer them. This set of questions has been reviewed and approved by the University of Toronto’s Office of Research Ethics. How will my privacy be protected? Protecting your information is our top priority. The following steps will be taken to protect your privacy: The survey will not contain any personal contact information except for your postal code, unless you would like to enter the raffle for a gift card in which case your email will be collected. Your data will be stored in a password locked folder on a university, password secured computer with bitlocker encryption so that only the researcher named on this consent form will access the data. When the project is over, the data will be destroyed to further protect your privacy. All publications or presentations resulting from this study will remove any information that can be used to identify you. Who can participate? You may be eligible to participate in this follow up if you: · Took part in the second round of our survey in March, 2021, and consented to receive an email about participating in a follow up survey. · Are over 17 years of age. · Used public transit in Vancouver or Toronto more than once per week prior to COVID-19 The survey team reserves the right to limit the number of survey responses that will be accepted. What are the benefits to participating? Your participation will help increase understanding of how public transit use has changed as a result of the COVID-19 crisis, helping to identifying better decisions for transit in the future. You will also be provided an opportunity to enter your email into a raffle for one of 10 $100 President's Choice gift cards. Who can I contact for more information?
