Please bring the recipient’s PRC ID/ PCP ID, an original copy of the Diplomate certificate, and photocopies of the Diplomate certificate. PCP Office is open Mon - Fri, 7 AM to 4 PM. 2. Via Express Delivery: (The scanned copy of the Dipl omate certificate must be sent to secretariat@pcp.org.ph ) a. If around Metro Manila, the recipient must book an express delivery to pick up the certificate. Delivery fee c/o the recipient b. For MM and provincial: Mail via LBC express. Fees CTC Copy: P50.00 per copy (First copy is FREE of charge) Mailing Fee: P200.00 Payment of fees shall be transmitted through the following: 1) Gcash Bills Payment ● Login to the Gcash App ● Tap BILLS ● Tap SCHOOLS ● Search for the Philippine College of Physicians ● Input details below o PRC Number o Your Mobile Number o Your Name o Amount: o Your Email Address To ensure a smooth transaction, please send us the receipt, recipient's name, address, contact number, and Gcash number after paying for postage. Old passers must first settle any outstanding annual dues upon request of the authenticated copy of the Diplomate certificate. You may reach the Membership and Credentials Committee at email membershipcom@p cp.org.ph II. Request for Temporary Certification in Passing the Diplomate in I.M. 1. Write a letter addressed to the incumbent secretary about the purpose of the request 2. Send the letter/email to psbim.2020 @gmail.com ( Email Subject: Request for Temporary Certification in Passing the Diplomate in I.M. [year passed ] ) 3. The certification letter must be approved by the Secretary prior to the release 4. The certification letter will be emailed to the recipient or picked up at the PCP office. ● A letter of authorization with an identification card attached must be shown in the owner's absence. Fees Certification: P500.00 per request Payment of fees shall be transmitted thru: 1. Indicated in the certification that they passed and are scheduled to be inducted and that certification is only temporary and will be used solely for the requested purpose. 2) Conferred Diplomates who are still in their post - residency deployment III. Request for PSBIM Result Certification Please note that PSBIM only provides Passed/Failed results, no scores 1. Write a letter addressed to the current PSBIM Chair about the purpose of the request 2. Send the letter/email to psbim.2020@gmail.com ( Email Subject: Request for PSBIM Result Certification [year ind ucted]) 3. The certification letter must be approved by the PSBIM Chairperson prior to the release 4. Request for a Member, Diplomate Certificates, PCP ID, and/or Pin 1. For Members & Diplomates inducted FY 2020, 2021, and, 2022 All certificates were delivered to respective PCP Chapters of those who submitted to PCP their authenticated/ certified true copy of their residency diploma. If you haven't submitted yet, please consider the following options: Important : A copy of your residency certificate and/or post - deployment residency certificate (if undergone DOH post - deployment) must be submitted prior to claiming Member and Diplomate certificates. 1) Visit the PCP Office and claim the certificate personally. Please bring the recipient’s PRC I D/ PCP ID. A letter of authorization with an identification card attached must be shown in the owner's absence. 2) If around Metro Manila, the recipient can book an express delivery to pick up the certificate. Delivery fee c/o the recipient; or ❖ For provincial and MM: Mail via LBC Express. The shipping fee will be shouldered by the recipient and to be paid before sending the documents. For Members & Diplomates inducted F.Y. 2023 All certificates, pins, and PCP IDs were distributed during the recent annual congress for those who completed and submitted the necessary requirements ahead of the deadline. (Requirements are the Letter of Intent, a Certified True Copy of your Residency T raining Program Diploma - for those who have not submitted it yet during the application - , and the proof of payment for FY 2023 - 2024 as a Diplomate) For those under the DOH - Residency Training Institutes , please be informed that you may only receive a copy of your PCP Diplomate Certificate AFTER you have submitted your Certified True Copy of your Residency Training Program Diploma ( not Certificate of Completion ) that is issued to you after your 1 - year deployment This must be submitted to karen.delrosario@pcp.org.ph cc: psbim.2020@gmail.com prior to claiming Member and Diplomate certificates.
