This survey is completely voluntary. There are no nega 8 ve consequences if you don’t want to take it. If you start the survey, you can always change your mind and stop at any 8 me. What is the purpose of this study? In order to gain a perspec B ve on the challenges faced by today’s college students and the impact those challenges have on teaching, I will conduct a survey of horn instructors in the United States and ask them about their experiences with altering and adap B ng curriculum, speci fi cally over the last 24 months. Risks • Some ques B ons may be personal or challenging to write about. You can skip them or quit the survey at any B me. • Online data being hacked or intercepted: Any B me you share informa B on online there are risks. We’re using a secure system to collect this data, but we can’t completely eliminate this risk. • Breach of con fi den B ality: There is a chance your data could be seen by someone who shouldn’t have access to it. We’re minimizing this risk in the following ways: o We’ll store all electronic data on a password-protected, encrypted computer. o We’ll keep your iden B fying informa B on separate from your research data in published research documents using pseudonyms and Subject ID's. You may wish to look at all the ques B ons before responding to help understand what kind of informa B on is being collected. Informa B on will be kept no more than one (1) month a Y er the fi nal Research Document has been submi 1 ed and approved. Who can see my data? • We (the researchers) will have access to all of the informa B on and answers submi 1 ed in the Google Form.
