Eligibility and Processing Details • Amount Available to Cash In : o You must have at least 24 hours of PTO available at the time the PTO will be paid out. Please consider any time being used during the pay period when the PTO is paid out when calculating your balance (future takings beyond the current pay period will not be factored in). You can check your PTO balance in UKG. o If your PTO balance is below 24 hours, you will not be eligible for PTO cash-in. o Future PTO requests may be cancelled if you do not have enough hours to cover the time off. Please ensure you will have enough PTO to cover future requests, after your PTO cash out. o If your requested amount will drop you below 24 hours, the request will be returned to you for adjustment. Please note: if you do not reply or re-submit the adjustment by the deadline listed below, the request will be processed in the next available pay cycle. o There is an annual cap amount available for cash-in based on your years of service. For specific limits, please contact your HR representative. If you have already met your PTO cash-in limit, you will be notified that any additional request cannot be processed. • Manager Approval o The request must be approved by your manager before submission to HR. o You are responsible for ensuring the request is submitted by the deadline. If you give the form to your manager, please follow-up to make sure it is sent to HR in a timely manner. Any forms received after the deadline, regardless of reason, will not be able to be processed until the next payroll cycle. • Deadline and Processing o Once your form has been approved and signed by your manager, you must submit it to Robin Hayden hr@orthoalliance.com by 4:00 pm on the Wednesday of the week prior to the pay date . This deadline ensures the request can be processed and included in the upcoming payroll cycle. Any forms received after this deadline will be processed on the next pay cycle available. o HR will verify that you have the required 24 hours of PTO available and submit the form to Payroll. • Receiving Your PTO Payout: o The PTO cash-in will be included in your upcoming paycheck. o The payout will appear on your pay statement with applicable tax deductions. o After the cash-in request is processed, your PTO balance will be adjusted to reflect the hours you cashed in. You can verify the updated PTO balance in UKG.
