Historic phthisis bulbi OS, ascites not grossly appreciable (20 December 2022), rear right limb stiffness (26 November 2026). Chronic bilateral otitis media first diagnosed ~27 years ago. Last otic endoscopy performed on 24 April 2026 using a 3mm STORZ bronchoscope via medium ear cone to maintain patency. Both ear canals improved since last bronchoscope, minimal discharge present and intact tympanic membranes. Keepers voice concern for persistent urine dribbling (R/O in musht/behavioral). Animal has been noted to have weakness in her stifles bilaterally by one keeper but this is not a consistent report. Keeper reported animal's tail due to its abnormal appearance and the elephant's history of ingrown hairs in that area. Otherwise, keepers report good appetite, behavior and activity levels. PHYSICAL EXAM Animal handling : Protected contact with voluntary participation and trained behaviors; uneventful Weight: 2,249 kg (27 July 2026) BCS: 5/9 (4 June 2026) General: Bright, alert, responsive. EENT: Eyes free of discharge OU; phthisical OS (historical); fundic exam not performed; nares are clear and free of discharge/debris bilaterally; mild white discharge present at ear canal opening AU with long hairs covering ear canal openings. Normal oral exam with normal wear of teeth. INTEG: No ectoparasites or dermal lesions appreciated; vertical fissures present on the nail of digit 3 and 4 of the right thoracic limb . Tail had multiple small nodular lesions; affected tissue flaky and came off when manipulated; had a malodorous odor. M/S: Ambulatory x4 without evidence of ataxia; well-muscled with adequate tone and symmetrical distribution. No weakness appreciated at brief standing exam. NEURO: Subjectively appropriate mentation and ambulation; no asymmetry of the face or musculature appreciated; full neurologic examination not performed. DIAGNOSTICS: Radiographic evaluation of all four feet, including dorsomedial-palmarolateral oblique (DMPLO) projections of the forefeet and dorsoplantar (DP) projections of the hind feet centered over digit 3 (D3) for all views. Awaiting radiologist interpretation. TREATMENTS: Rabies vaccine: Administered Elanco Rabvac (E206790A, exp 8 Sept 27) 1ml IM in R rear limb following chlorohexidine scrub and alcohol wipe. Tetanus vaccine: Administered Zoetis tetanus toxoid (797734A, exp 12 Feb 27) 1mL IM in R front limb following chlorohexidine scrub and alcohol wipe. ASSESSMENT 1 Urine dribbling: R/O musth-associated hormonal changes, urinary tract disease, incontinence, urolithiasis. 2 Nodular tail lesions: R/O ingrown hairs/folliculitis, chronic trauma, papillomas, neoplasia. 3 Rear limb stiffness/weakness.
