Pactiv 10 Critical Safety Rules (CSR) 1) Failure to follow lock - out/tag - out procedures. 2) Reaching into an operating grinder, shredder or chopper 3) Failure to use appropriate blocking devices where required. 4) Physical contact with a nip point of any operating equipment such as rollers or conveyors 5) By - passing or disabling any safety guard , interlock, e - stop or similar device (outside of a LOTO procedure) 6) Unauthorized entry (any “breaking the plane”) into a permit - required confined space 7) Smoking anywhere except in authorized and designated areas. 8) Reckless operation of a powered industrial truck or other motorized vehicle 9) Failure to secure a trailer prior to entering as a pedestrian on foot or with mobile equipment. 10) Performing other unsafe acts that present serious risks to life and property as defined by management. **PACTIV LONDON, ONTARIO** • London Plant makes single use cups for hot drinks and fries. • Key Customers – Tim Hortons, Starbucks and McDonalds. • We have receiving, printing, cup forming and shipping departments • We are sharing space with lift trucks. There are approved walkways for pedestrians. We installed railings to separate lift trucks routes and pedestrians. To ensure safety of the pedestrians at cross point with lift trucks routes we installed gates, globe mirrors and STOP signs for pedestrians and lift truck. Our trucks are equipped with blue and red lights to enhance the visibility of the trucks. • Allowable break are: four 10 min break and 20 min lunch. Smoking is allowed only in the patio area at the front of “Employee Entrance”. GMP Policy • All persons working in direct contact with products, product - contact surfaces and production equipment are required to conform to the following hygienic practices while on duty: • Any person who, by medical examination or supervisory observation, is shown to have, or appears to have, an illness, open lesion, including boils, sores, or infected wounds shall be excluded from any operations which may lead to product contamination. FRM - 20QA - TRN - 013 5 6 Scope of Work • Before work begins, Constructors or General Contractors are expected to: • Verify WSIB clearance • Ensure all workers are trained and currently certified in the types of tasks related to the project such as; • Working at Heights, EWP/AWP • WHMIS • 4 - step/5 - step OHSA • ESA electrical • Respirator fit • Forklift or mobile crane • Lock - out/tag - out • Based on the size of crew, constructors or GC must ensure adequate supervision is present throughout Proper Attire No Hoods ▪ Hoods can get caught on/in machinery, and it blocks forklift drivers peripheral view! No Strings and No Baggy Clothes ▪ Dangling strings on jackets, shirts and pants can get wrapped into spinning shafts or gears! ▪ It is recommend that shirts be tucked in to avoid injury from turning equipment. Hooded Sweatshirts NOT allowed Good Manufacturing Policies (GMP) ▪ Clothing Requirements • Shirts and blouses must have sleeves • Do not store items in pockets above the waist • Clothing must be in clean and good condition • Clothing cannot have adornments such as rhinestones, glitter, etc. • Loose fitting clothing or clothing with hoods and/or drawstrings are prohibited. Employees cannot tie clothing around their waist 8 Proper Attire No Backpacks or Bags ▪ Tool boxes are permitted, but no backpacks, satchels, or bags No Food or Open Drink Containers • Sealed water bottles are permitted, but food consumption and open top drink containers must be kept to the break areas and offices PPE Required in Production Areas Personal Protective Equipment (PPE Different Types of PPE include: ▪ Safety Footwear/Steel Toe ▪ Safety Glasses/Goggles • Goggles required when using compressed air ▪ Hearing Protection ▪ Bump Caps ▪ Gloves for cut/burn protection ▪ Sleeves for cut/burn protection PPE is a requirement in our facility and not optional. Pactiv will supply all required PPE except for steel toed shoes and prescription safety glasses. Glove Policy ▪ To keep your hands and fingers safe, gloves are worn for many tasks. ▪ Your Job Safety Assessment will tell you when and where to wear gloves. ▪ You may also use this chart for examples of what gloves to wear. ▪ A good way to keep your gloves close is with one of our glove clips. GMP (Good Manufacturing Policies) ▪ Hair Restraints • All employees are required to wear hairnets while in the facility. - White hairnets are for full time employees, green are for temp employees and red are for visitors and contractors. - Colored bump caps can be used in place of colored hairnets. • Beard nets are also required for facial hair other than a moustache that does not extend past the top lip. • All hair must be contained inside of the hairnet. - Some people may require additional hairnets depending on hair volume and style. Why we Enforce GMP • All of our products contact food. • Contamination with foreign matter must be prevented. • Public safety is at risk. • Our customers demand it. FRM - 20QA - TRN - 013 Good Manufacturing Policies (GMP) ▪ Jewelry Restrictions • Jewelry is not permitted to be worn on the floor. • Jewelry includes earrings, finger rings, watches, fitness monitoring devices, necklaces, nose rings and any other exposed piercing. • Fingernails must be clean. No false nails, polish or glitter is allowed. • Fake eyelashes are not permitted. • Tattoos less than 14 days old must be covered. • Henna Tattoos must be covered at all times. • Clothing must be clean and close fitting. • No personal belongings on the floor (including lotions, cosmetics or personal medicines). • No personal cell phones. • No bandanas or baseball caps. • Employees that are sick, have infected cuts, boils or sores, skin conditions, or diarrhea, must not handle product. To Avoid Product Contamination • Wash Your Hands • Before Starting Work • After Break • After Lunch • After Using the Rest Room • After Touching the Floor • After Leaving the Building • Brush off your clothes to remove crumbs • Wheat in bread is an allergen FRM - 20QA - TRN - 013 GMP – Hand Washing Hand washing must be conducted on entry to a production facility, after using the bathroom, eating and smoking or when hands become soiled, to prevent contamination to product. Hand and Finger Injury Prevention ▪ Never reach into an operating piece of equipment ▪ Never place your hands or fingers where you can’t see them! ▪ Never wear gloves near moving/ rotating equipment ▪ Follow all procedures when using a box taper • Clearing a stuck box/case • Tape/Roll Changes • Get an authorized person to assist you if you have not been trained 18 Lock Out/Tag Out - LOTO Control of Hazardous Energy ▪ What is Lock Out/Tag Out? • A procedural system that requires systematic control of energy through use of locks and tags on energy isolating devices. ▪ Lock Out/Tag Out is used to protect employees from the hazardous energy and unexpected start - up of equipment during servicing and maintenance activities. Lock Out Tag Out 20 ▪ Lockout is defined in the Canadian standard CSA Z460 - 20 "Control of Hazardous Energy - Lockout and Other Methods" as the "placement of a lockout device on an energy - isolating device in accordance with an established procedure." A lockout device is "a mechanical means of locking that uses an individually keyed lock to secure an energy - isolating device in a position that prevents energization of a machine, equipment, or a process."
