I/the Applicant hereby confirm that I/the Applicant has never applied for debt counselling/business rescue/any compromise wit h creditors and that I/the Applicant am solvent. I/the Applicant acknowledge that, in line with the Protection of Personal Information Act, 4 of 2013 (POPI), I/the Applicant have been adequately informed of my/our rights and purposes for which any personal information provided to the Credit Provider may be processed. I/the Applicant hereby declare that the information provided by me is true and correct and I am duly authorised to act for an d on behalf of the directors/members/trustees/shareholders/beneficial owners (whatever the case may be) of the Applicant.
