Use this sheet to record a battery or charging complaint before anyone attempts a technical check. It helps separate a stop-use concern from missing information. A completed sheet does not certify safety, remaining capacity, compatibility or fitness to fly. 1. Choose the evidence outcome STOP USE A pack is swollen, deformed, leaking, damaged, unusually hot or has an abnormal odor. Impact, corrosion or moisture concerns also need qualified assessment. Do not charge, fly, open, puncture, repair or bypass protections on a suspect pack. Avoid handling it if doing so may be unsafe; follow manufacturer and applicable local guidance. HOLD FOR INFORMATION Exact battery identity, charging equipment, warning or event history is missing. Record what is known and what remains unknown. A similar-looking cable or connector does not establish a supported charging path. REFER TO MODEL INSTRUCTIONS No stop-use concern has been identified and the exact equipment can be matched to its manufacturer documentation. Use that documentation and appropriate qualified assistance to decide the next step. This worksheet itself does not authorise a charge test. Keep observations separate from conclusions Write the exact indication and when it occurred. "Three LEDs flash together" is more useful than "the battery is dead". Record a pre-existing, supported observation without repeating a test just to complete this sheet. Leave unknown fields marked unknown Example: another battery working normally does not clear a visibly swollen pack for testing. Exterior appearance alone cannot establish internal condition. Record only observations that can be obtained without creating an unsafe situation. Record date and local case reference Aircraft model and battery label Exact model / part identifier; mask serial numbers if sharing. Charging route Direct connection or hub; exact adapter, cable and power source. Exterior concerns Known swelling, damage, odor, heat, impact, corrosion or moisture concerns. Exact indication already observed LED pattern, application warning or error text; when it appeared. Event and storage history Last normal use; storage; impact, moisture or interrupted-charge history. Manufacturer reference Document title / version and the relevant model-specific section. Previous supported observations Who observed what, when and with which equipment. Do not repeat unsafe tests. Practise the decision The three cases below are hypothetical learning exercises. They are not customer cases or evidence about any battery model. Case A A pack is visibly swollen. A second pack works with the same hub. Which outcome applies, and what does the second pack fail to establish? The owner has mixed cables and adapters from several kits and cannot identify them. What information is missing before a charging symptom can be interpreted?
