Keepers reported elephant is not bothering with area, eating and acting normally; normal ambulation. Today the same stopper was reapplied to the area after gauze change. Stopper removed rather easily with flat head screw driver in the middle hole of the stopper. Outside 4x4 gauze had moderate sand and was grey in color. The piece of Kerlex AMD gauze was slightly dirty with sand but the aspect of the gauze that has contact with the tissue was clean and white in appearance. Did not smell and was dry to the touch. Applied one 3x3cm Kerlex AMD roll gauze (size is approx.) damped (but not dripping) with hypertonic saline (5-6 mls) deep into the wound using surgical tool and digital pressure - uneventful Followed by packing one 4x4 clean but non-sterile gauze afterwards before the stopper was replaced. Stopper application: Used stopper number 2; took some time and effort (2 people) to insert it and keep it in place; overall elephant tolerated the maneuvering of the area w/o much reaction. Hx: Keepers reported in am that rubber stopped had gone inside the wound. In closer inspection the stopper had NOT gone into the woud but the outside portion of the 3 piece stopper had detached and only the center (covering the hole) and the inside lid were present. Keepers reported elephant is not bothering with area, eating and acting normally; normal ambulation. After morning recheck it was decided that stopper could stay in place until afternoon procedure. Stopper change: Old stopper (mid and internal sections) removed rather easily with car tools and small flat screwdriver Outside 4x4 gauze is dirty with mod amount of sand. Since wound has been managed as open, it appears to be contaminated but not infected. Quick debridement procedure performed on 22July2026 to remove necrotic tissue and widen lamellar ring/circumference; depth measured at approx. 2.5cm at that time which is significantly decreased from initial. Care staff reports that elephant seems to be doing well overall, but that the results of packing are inconsistent regarding finding gauze within the wound during morning checks (i.e. some days keepers find the Kerlex, other days they find 2 gauze (out of 3), but most days they do not find any remaining gauze within the wound). Current therapies: Ibuprofen 20,000 mg BID PROCEDURE : Step 1: External cleaning, debridement, and trim Elephant positioned in Hallway 1 with swing gate closed and front right limb intermittently tethered (allowing breaks throughout). The right front foot was presented by resting on the lowest horizontal beam of the swing gate. Packed gauze was removed and wound was flushed with cool water from the hose (at a low pressure). Necrotic tissue was debrided from the wound using a Ruskin rongeur; mild active hemorrhage noted. Nail was then trimmed and the circumference of the wound was widened slightly by trimming the lamellar ring. Wound was then flushed with hypertonic saline. Step 2: Apparatus template and placement Ensured appropriate conformation of nail prior to creating template for new apparatus. Dry gauze packed within wound for temporary cover while apparatus was being made. Template created in similar manner to previous - urethane horse shoe patch was placed on the bottom of the toe and permanent marker used to trace the outline of the solar surface of the nail. This apparatus was modified to include a large center hole, approximately the size of the wound (cut using a 35mm round saw) to accommodate a custom stopper. Custom stoppers pre-made from three layers of urethane and welded together; center layer equal in circumference to center hole in apparatus (35mm). The outer layers of the stopper have a larger circumference than the center hole. The stoppers are flat on the internal surface and slightly convex on the outer/plantar surface and have a center hole to aid in removal from the apparatus. Excess acrylic from previous apparatus applications removed from the nail. Surface of nail prepped with wire wheel connected to power drill and buffer powertool. Surface of nail wiped clear with alcohol to remove excess debris. Dry gauze remained within wound during application of the apparatus. Farrier SF applied apparatus to plantar surface of nail with Equilox II acrylic ensuring to avoid center of apparatus (to avoid application of acrylic within the area of wound/center hole); apparatus had 5 pre-drilled holes around periphery to facilitate adherence of the acrylic to the nail. The entire apparatus, with the exception of the center drainage hole was then covered with acrylic, smoothing and feathering the edges along the front edge of the nail and the solar surface. After secondary layer of acylic had cured, super glue was applied to seal any small defects. Hair dryer was used on low temperature setting to speed curing process. Dry gauze was removed and the wound was packed with one ~3x3 Kerlex AMD roll gauze square moistened with hypertonic saline, followed by one 4x4 non-sterile dry gauze square. Custom stopper positioned within center hole to seal wound. Hx: Keepers describe this elephant as doing well since the apparatus was removed and daily gauze changes have ocurred (see below* for specifics). Keepers report elephant is walking normally and not keeping the RF limb up like before. *Note : Keepers are checkng the wound in am and removing dirty gazues (if present) and inserting two clean 2x2 gauze in the wound. Finding gauze inside the wound is not consistent; some days keepers find the Kerlex (deep one) and some days they find 2 gauze (out of 3) and most days they do not find a gauze inside the wound in am. Veterinary staff is performing the guze change in pm when applying the Kerlex AMD gauze inside the wound. Observation from the past several days: The wound appears to be contaminated but not infected; the most external portion of the tissue is darker in color but passing the initial wound nail rim the tissue appears with healthy color. Yesterday it was noted that the depth of the wound was subjectively reduced and also the nail was starting to close. It was decided that in ordre to continue treating with BID gauze changes the nail rim needed to be slightly opened and some of the presumptively necrotic (dark) tissue removed. Overall the BID gauze change has worked well with animal care staff peforming the changes with non-treated gauzes in am and veterinary team performing the medicated (Kerlex AMD) gauze inside. Measured the dept of the wound today: 2.4cm (reduced with original 6cm in depth and even last measurement during the last apparatus removal (3cm). Wound has a small amount of sand. Wound care: Wound gently flused with tap water for removal of sand - by keepers uneventful Dried the external portion of the wound with dry gauze - uneventful Debridment and biopsies of what appears abnormal tissue (dark tissue) and deep granulation tissue. Rongeur used today for biopsy samples was a Ruskin Rongeur, a double-action surgical instrument featuring a 7-1/4" (18.4 cm) length and straight 7 mm bite jaws. This Rongeur is designed for orthopedic, neurosurgical, and veterinary procedures to cut or scrape bone. Manufactured by Miltex code 19-854. This tool worked very well for intended purposes today. Mild to mod bleeding occurred from the biopsies obtained from deep granulation tissue; elephant resented this portion of the procedure but able to finish procedure with positive reinforcement; bleeding controlled with gentle digital pressure with a gauze. Photo obained after debridement and bleeding had been controlled - saved to Alex After debridement the routine protocol for wound care (as outlined above and below) was performed uneventfully Samples collected: - Culture from deep in the wound for aerobic and anaerobic testing at OVDL - 4 biopsies obtained as described above for histologic evaluation at ZPP - UofI Samples divided as follow: 3 samples for the first cassette, 1 sample in the second cassette. 4 total in formalin 10% A: Chronic abscess on RFD4; currently treated with BID gauze changes with the pm change using a medicated gauze. Today debridement appears to show healthy granulation tissue (deep wound) and contaminated laminar tissue - samples submitted to lab for evaluation P: Continue with BID gauze changes; am performed by keepers and PM by veterinary staff. Until now, only veterinarians were performing procedure but vet-techs will be integrated as of today (see email below). Evaluation: Visual performed after keepers have rinsed area with a gentle stream of tap water to remove the sand. Tissue inside wound presents 2 different appearances, the white and light pink deeper in the wound (R/O granulation tissue) and darker (greenish) tissue closer to the external aspect of the wound (R/O necrotic tissue, both soft tissue and laminae tissue). Treatment and gauze change: No gauze observed to have stayed over night or in pm. A 3x3cm piece of Kerlex AMD roll gauze damped in sterile hypertonic saline was packed as deep as possible, elephant reacted and pulled foot back but gauze remained inside. Two 2x2 cm square gauze packed after initial gauze for a total of 3 pieces inside wound. A: Continous treatment consisting of packing wound with Kerlex AMD and dry gauze for now until new apparatus desgined has been refined; procedure for debridement scheduled on July 22nd; evaluate tissue and consider starting treatment for granulation if infection has been controlled. P: Cont with treatment with daily (BID) changes of gauze. Treatment at 2:30 pm. Hx: Since July 3, 2026 when the plantar apparatus was removed the wound has been cleaned twice with tap water, once a day packed with dry 2x2 gauze, and once a day with Triz-EDTA soaked gauze. Some days both gauzes remain inside overnight, sometimes only 1 gauze is removed (by keepers in a.m) and sometimes no gauze is found inside the wound. In consultation with Dean. A. Hendrickson DVM, MS, DACVS it was decided to switch wound management to daily changes (SID, BID or even TID) for more aggressive infection control. For this he recommended the use of hypertonic saline soaked Kerlex AMD (antimicrobial coated gauze) inside the wound to get the infection under control. Hypertonic soaked Kerlex first applied 13 July 2026. Changed 14 July 2026 at 2:30pm. Today wound inspected and hypertonic Kerlex gauze changed again. Wound inspection: Wound has been open since July 3, 2026 with keepers doing daily cleaning, removal of contaminated gauze and insertion of two 2x2 gauze inside the wound. Wound appears static from previous visuals; there is a light pink tissue deep into the cavity (approx 3-5cm depth) and greyish/greenish tissue around the wound entrance (R/O stained tissue vs necrotic tissue). Overall static visual to recent previous exams. Photos obtained on 13 July 2026, none at today’s gauze change. Procedure: A piece of approximately 3x3 inch square shape was sectioned from the Kerlex AMD roll and soaked (but not dripping soaked) in hypertonic saline (about 5mls); once wet it was introduced deep inside the wound with finger (mild reaction) followed by two 2x2 square dry gauze; elephant resented the Kerlex gauze inside the wound but procedure accomplished uneventfully. A : Wound infection, currently trying to keep infection under control before deciding how to promote granulation prior to letting wound close. Treatment at 2:30 pm Hx: Since July 3, 2026 when the plantar apparatus was removed the wound has been cleaned twice with tap water and in once a day packed with dry 2x2 gauze and once a day with Triz-EDTA soaked gauze. Some days both gauze remain inside overnight, sometimes only 1 gauze is removed (by keepers in a.m) and sometimes no gauze is found inside the wound. In consultation with Dean. A. Hendrickson DVM, MS, DACVS it was decided to switch wounc management to daily changes (SID, BID or even TID) for more aggresive infection control. For this he recommended the use of hypertonic saline soaked Kerlex AMD (antimicrobial coated gauze) inside the wound to get the infection under control. Today is fhe first day that this Kerlex AMD gauze will be used. Note : Elephant staff asked again about the use of CO2 in the wound; this has been addressed about 1 1/2 year ago but same response was given to the staff: Theres is no evidence that use of CO2 in elephant wounds and specifically toe abscesses has resulted in any benefits; the few animals that have "resolved" were more related to observational biasses in institutions with limited veterinary coverage (observations mostly from keeper staff); institutions that have used and were willing to share information when a thorough literature search and consultation with AZA institution veterinarians yielded no scientific evidence of benefits and literature suggest the CO2 will negattively affect healthy tissue. All the evidence found was only empyrical and mostly driven by company and or keepers that have "seen results". Vet tech KFlan was at MCZoo when this therapy was used in an elephant at the recommendation of consultant MMc, the assesment from vet team after use was that it was very expensive and did not yield any significant positive results. Visual: Elephant observed from the 2nd floor and 1st floor during ambulation: Elephant is subjectively putting more weight on RF limb and even putting full weight on limb when resting. Wound care: Lesion visualized after keepers have cleaned with water; wound is clean and mostly devoid of sand and debris. Dried out with dry gauze and placed two gauze inside the wound, the 1st one wet (not soaked wet) with the Triz-EDTA and the 2nd dry to serve a barrier. Used index finger to push the 1st gauze all the way up and the 2nd one was packed to serve as barrier; elephant lightly resented the push of the 1st soaked gauze.
