1. Safety Responsibilities Supervisors are responsible to ensure : • That a Safe Work Permit is issued and Hazard assessments are performed prior to starting the work activity; • The health and safety of workers and visitors on the work site; • That workers are properly trained to perform their job and aware of their responsibilities; • Compliance with government regulations, industry standards, and company guidelines; • Appropriate too ls and safety equipment are supplied, used and maintained in a safe manner; • Workers use required safety and personal protective equipment; • Unsafe conditions/acts are to be corrected immediately; and • COVID 19 Safety Protocols are followed; • Workers are infor med of what is expected of them. Workers are responsible to: • Comply with requirements of Safe Work Permit; • Protect the health and safety of themselves and other personnel on site; • Properly use and maintain personal protective equipment; • Abide by all government regulations and company policies, procedures and standards; • Report accidents and potential hazards to their supervisors • Refuse to work under unsafe conditions or perform work for which they are not adequately trained; and • Correct unsafe conditions/acts immediately Advantage Oil & Gas Ltd. is committed to ensuring its employees and contractors of their right to work in an environment free from discrimination or harassment. 2. Safety Standards Alcohol, Cannabis and/or Illegal drugs • Workers under the influence of alcohol , cannabis or illegal drugs are not permitted on company premises. Violators of this policy will be removed from the site; and • Workers under any prescribed medication that may impair their ability to work must notify their supervisor. Confined Space Entry • M ust be performed according to Alberta Occupational Health and Safety Code, Part 5, Confined Spaces and to company Codes of Practice and Safe Wo rk Procedures. Cranes and Hoists • Must be operated by properly trained and certified personnel; • Workers must not stand on or under suspended loads; and • Tag lines must be used and hooks must be equipped with safety latches. Electrical Operations • Must be pe rformed by trained or certified personnel; • Equipment must be properly locked out and tagged before electrical maintenance can be performed; and • Workers on equipment must maintain appropriate distances between power lines and their equipment. Contact power company representatives to confirm clearance requirement. Excavations and Trenches • Excavations exceeding 1.5m depth cannot be entered unless they are protected from cave - ins or sliding material (proper shoring or trench cut back) (Sask & BC Regs only allow 1.2m); • Locations of underground pipelines, cables, and conduits must be identified before digging; • Buried pipelines and cables must be hand exposed or day lighted before mechanical excavation is permitted. Follow Company Ground Distu rbance Code of Practice and/or Industry Standards; • Adequate access and egress must be provided for all excavations; • Mechanical equipment is not permitted within 60cm of energized pipelines and electrical cables; • The spoil pile must be at least 1m from the edge of the trench; and • Excavations and trenches must be adequately marked or barricaded to prevent accidental entry by personnel, livestock or wildlife. Fall Arresting Equipment • Must be worn by all workers working 3m above any platform or work station. Ha ir • Must be completely confined or cut short where there is a danger of it contacting moving equipment; • Beards, goatees, or excessive facial hair are prohibited on worksites where hydrocarbons are present or could be released as SCBA/SABA units may be requi red; and • Jewelry is not recommended. Housekeeping • Work areas must be kept clean and free of any unnecessary obstructions, and • Spilled toxic or corrosive chemicals must be cleaned up immediately and all wastes must be disposed of according to government regulations. Ladders • Must be inspected before each use for broken or damaged rungs, broken side rails, or missing no - slip devices; • Ladders must be secured at the top, or otherwise held in place, to the structure they are set against; • Employees working from a fixed ladder at 3m or more above a platform must wear a safety harness secured to a suitable structure; and • Workers are not perm itted to work from the top two rungs of a portable ladder Lockout/Tagging and Isolation • All valves or sources of energy must be locked out and isolated if their accidental opening or closing will create a hazard to workers; • Every worker conducting maintena nce or repairs must lockout/tag the equipment they are working on, using a lock that is operated by only one key; • The worker must retain possession of that key while they are working on the equipment; and • The worker must remove the lock/tag when they are f inished the repair or maintenance work. Machine Guards • Guards are provided to protect personnel from moving parts on machinery and must be kept in place at all times; and • Hand held grinder must have adequate guards in place at all times. SMOKING IS ONLY PERMITTED IN DESIGNATED AREAS. 3. Health/Safety Hazards • A hazard assessment must be performed prior to commencement of work to identify potential risks; • All diesel vehicles must have an operable automatic positive shut off. A manual shut off is acceptable as long as the vehicle is never left unattended while running; • Some of the substances you may be exposed to while working at these facilities can be harmful to your health; • Personal p rotective equipment is required when working with Asbestos; Benzene; heavy metals, Hydrogen Sulphide; pesticides, radioactive material, solvents, acids etc; and • Refer to MSDS found at facility offices. 4. Safety Training • First Aid/CPR – All Personnel; • WHMIS – All Personnel; • TDG – All Personnel who receive, transport, offer for transport, or receive dangerous goods; • H2S Alive – All personnel who could be exposed to H2S or Oxygen deficient atmosphere. • Confined Space – All Personnel who could be working in a confined space; • Service Rig BOP – Service Rig Supervisor • Drilling BOP – Driller and Second Line Supervisor; • Fall Protection/Rescue – All personnel exposed to a falling hazard in excess of 3m; • Driving License – All personnel; and • Ground Disturbance – All personnel who are supervising and performing excavation operations below 30cm. 5. Personal Protective Equipment • All workers are worksites must wear the following personal protective equipment; • ANSI or CS A approved hard hats • CSA approved safety - toed footwear appropriate to the work being done; • Flame resistant outer work wear when working on any site or location where hydrocarbons are present or could be released; • Nylon clothing is prohibited in any area th at has hydrocarbons present; • Safety eyewear that comes with side shields must be worn at all times and be appropriate to the work being done, (face shields for grinding and lighting burners, goggles and a face shield when handling chemicals); • Hearing protection in areas where noise levels exceed 85 dBA; and • CSA approved safety harness and lifelines when working more than 3m above the ground or more that 3m above a permanent worksite platform 6. Emergency Procedures • All workers must be familiar wi th the Site Specific Emergency Response Plans and the supervisors will be familiar with the Corporate Emergency Response Plan; and • The most senior employee on site will notify their supervisor and request assistance if an incident/accident occurs 7. Emergenc y Equipment • All workers must be familiar with the location of fire extinguishers, eye was stations, MSDS, the first aid station, and any other emergency equipment on site. 8. Safe Work Permits Permits are used to ensure all personnel are aware of the scope o f the job, and that hazards and safe operating procedures are identified. Types of work where permits are required include: • Confined Space Entry; • Ground Disturbance; • Hot Work (welding/cutting/grinding/electrical) • Pressure testing; • General construction work; and \ • Any new work performed by a contractor. If conditions change from those outlined in the permit, the permit become invalid and all work must stop until a new permit and hazard review is prepared. 9. Safety Meeting s • Pre - job safety meetings must be held prior to undertaking a hazardous or potentially hazardous activity; • Ensure everyone on lease understands the work to be done, haz ards they may encounter, and safety precautions to be taken; • Document the meetings separately or as part of the permit; and • Document attendance records or all personnel at meeting. 10. Incident/Accident Reports • All incidents/accidents, injuries and near misses must be reported to the site supervisor; • All injuries must be recorded in the First Aid Log Sheet; and • If a first aid attendant is on site, all injuries must be reported to the attendant WORKSITE SAFETY ORIENTATION 10 Questions to Answer Before you Start Work. 1.
