requirements and processes for reporting, notifying, classifying, and investigating incidents on the project, in line with the Expo Riyadh Construction Safety Manual (ERCSM) and the ERC Incident Reporting and Investigation Procedure. This document describes how ABYATONA shall deal with illness and injuries in the workplace that are not covered in its Emergency Response Plan. 2 SCOPE This Plan applies to: ● Abyatona (as Contractor) and all Subcontra ctors ● All site offices, temporary facilities, and work areas ● All phases of the project (design, mobilization, construction, commissioning, and handover) ● All work - related injuries, illnesses, near misses, unsafe acts, and unsafe conditions 3 OBJECTIVES This Plan is intended to: ● Ensure that all incidents are reported, recorded, and investigated in a manner proportionate to their severity and level of risk. ● Provide a consistent classification system (P1, P2, P3) for incidents across all Abyatona sites. ● Ens ure immediate priority is given to preventing further harm and providing care to injured persons. ● Define clear notification and reporting timeframes to the PMC and Employer (ERC). ● Ensure root cause analysis and corrective actions are identified, tracked, a nd closed out. ● Promote a culture in which reporting is seen as part of learning and continuous improvement, not blame. 4 DEFINITIONS AND CLASSIFICATION ERC classifies incidents into three severity categories to ensure a consistent approach to incident mana gement across all sites, as summarized below. Level Criteria P1 (Major) ● A single or multiple fatality , or permanent disability of 3 or more persons. ● Serious injuries resulting in long - term physical impairment (limitation of physical functioning, mobility, dexterity, or stamina). ● An incident resulting in a complete stop of work for more than 1 week. ● An incident initiating a regulatory investigation requiring involvement of ERC Top - Level Management and possible seizure of ERC documents. ● Requires urgent repor ting to the Top - Level Management (CEO) of ERC. P2 (Medium) ● Hospitalization of more than 3 serious casualties with injuries that have potential long - term effects (e.g. broken bones requiring months to heal). ● Permanent disability of fewer than 3 persons. ● Lost Time Injuries resulting in 5 or more consecutive lost workdays. ● Requires disclosure to the regulatory authority and may require involvement of ERC Top - Level Management. P3 (Minor) ● Several minor casualties classified as Restricted Work, Medical Treatm ent, or First Aid Cases. ● Lost time incident resulting in fewer than 5 lost workdays. The table below defines the categories of incidents and events referred to throughout this Plan, together with notes to support consistent reporting, investigation, and tracking. Injury and Illness Management Plan Page 7 of 30 Classification Definition Notes Reportable Incident An incident that must be recorded and reported individually and in regular metrics reports. ● All reportable incidents must be investigated proportionate to the severity and risk level. ● All reportable incidents are to be recorded and included in regular metrics reports. Recordable Incident An incident that must be recorded and tracked in regular metrics reports. ● A simpler investigation is appropriate for recordable incidents, proportionate to the severity and risk level. ● TRIR is calculated as the total number of recordable incidents x 200,000, divided by total hours worked in the reporting period. Fatal Incident A work - related incident or ill - health resulting in the death of one or more per sons. ● Includes the death of any person(s) where the work activity contributed to the incident. ● Includes deaths due to ill - health where direct work - related factors may have contributed. ● Excludes road traffic accidents outside project boundaries or on public roads. ● Excludes fatalities from natural causes unrelated to work. ● Includes deaths occurring within one calendar year of the incident, or first manifestation of ill - health, where work - related factors may have contributed. ● Excludes suicide. High Potential Event (HiPo) An incident that had the potential to cause death, serious injury/illness, or extensive property damage, but these consequences did not occur. ● Any event that could have led to a fatality, Lost Time Injury, or property damage of SAR 300,000 or more. ● Any event or failure in critical control measures relating to: mobile plant and equipment, driving, work on/near live roads, breaking ground and excavations, work at height, lifting, confined spaces, fire, hot works, energized systems, temporary work s, drilling and blasting, work on/near water, and working in the heat. Lost Time Injury (LTI) An incident or ill - health resulting in a person being unable to work for one or more full workdays or shifts, not including the day of the incident. ● If the incid ent also meets a P1 or P2 classification, the higher classification applies. ● Includes rest days, weekends, scheduled holidays, public holidays, and days after cessation of employment. ● LTIR is calculated as the total number of lost time incidents x 200,000, divided by total hours worked in the reporting period. Restricted Work Case An incident or ill - health resulting in a person being unable to carry out normal work but able to perform alternative duties. ● Restricted Work Cases are not included in the LTIR c alculation. Medical Treatment Case A work - related injury or illness requiring medication, treatment, or medical review administered off - site by a healthcare professional, beyond first aid, without Injury and Illness Management Plan Page 8 of 30 Classification Definition Notes resulting in lost time beyond the day of injury. Occupational Illness / Disease An illness or disease caused or worsened by work activities, as diagnosed by a medical doctor. ● Includes occupational dermatitis, hand - arm vibration syndrome, occupational asthma, and carpal tunnel syndrome. ● Includes any occup ational cancer or disease attributed to occupational exposure to a biological agent. First Aid Case An incident requiring only first aid, with no resulting inability to perform normal work beyond the time taken to administer first aid. Near Miss An incident with the potential to cause harm or property damage/loss, but of lesser potential than a High Potential Event. ● No injury or property damage/loss was sustained. ● Near Misses may be Reportable or Recordable. During the initial site preparation, workers may be exposed to potential hazards such as uneven terrain, loose soil, and debris. Earthworks and excavation activities pose risks of slips, trips, and falls. The use of light 4 - wheel drive vehicles and tracked plant introduces the risk of vehicle - related incidents, including collisions and overturns. Additionally, the presence of geo - hazards may expose workers to the risk of encountering unstable ground conditions. Workers inv olved in the construction of ERC complex face similar risks, emphasizing the importance of adhering to safety protocols, proper training, and the use of personal protective equipment (PPE) to mitigate the potential for injuries and illnesses throughout the project's various phases. Regular monitoring and maintenance of equipment and access routes are crucial components of maintaining a safe work environment. 6.1 Slips, Trips, and Falls: Uneven terrain during initial site preparation and earthworks may lead to slips, trips, and falls. Icy or wet conditions, especially during cold weather, can increase the likelihood of accidents. 6.2 Vehicle - Related Incidents: Operating small 4 - wheel drive vehicles and tracked plant. Wide pioneer tracks pose risks of collisi ons, overturns, and mechanical failures. 6.3 Musculoskeletal Injuries: Manual handling during earthworks and excavation activities may contribute to musculoskeletal injuries if proper lifting techniques are not followed. Injury and Illness Management Plan Page 12 of 30 6.4 Geo - Hazard Exposure: Workers ma y face risks associated with exposure to unstable ground conditions when dealing with geo - hazards, including the potential for landslides or collapses. 6.5 Equipment - Related Injuries: Operation of heavy machinery, especially in challenging terrains, increa ses the risk of crush injuries, entanglements, and other equipment - related accidents. 6.6 Chemical Exposure: Clearance of geo - hazards may involve exposure to hazardous substances, necessitating proper handling and protective measures to prevent chemical - re lated illnesses. 6.7 Infectious Diseases: Working in close proximity to multiple workers on the construction site may increase the risk of infectious diseases. Adequate sanitation and hygiene practices should be maintained to mitigate this risk. To address cold weather injuries specifically, it is crucial to implement measures such as providing appropriate cold - weather clothing, conducting regular safety briefings, and monitoring weather conditions closely. Workers should be trained on recognizing the sympt oms of cold stress and encouraged to take breaks in heated areas to prevent adverse health effects. 6.8 Heat Stress : Workers exposed to high temperatures for prolonged periods may experience heat - related illnesses, including heat cramps, heat exhaustion, heat stroke, and dehydration. Control measures include implementing the Heat Stress Management Plan, monitoring weath er and heat index, scheduling work to avoid peak heat periods where practicable, providing shaded rest areas, ensuring an adequate supply of cool drinking water and electrolytes, encouraging regular hydration, allowing frequent rest breaks, acclimatizing n ew workers, training personnel to recognize the signs and symptoms of heat stress, and ensuring prompt first aid and emergency medical assistance for affected workers. 6.9 Dust Exposure : Workers may be exposed to airborne dust generated during excavation, demolition, cutting, grinding, drilling, backfilling, and vehicle movements, which may cause respiratory irritation, breathing difficulties, coughing, and eye irritation. Control measure s include implementing dust suppression using water spray or misting, covering stockpiles and transported materials where required, maintaining good housekeeping, minimizing dust - generating activities, providing suitable respiratory protective equipment (R PE) and safety goggles, monitoring dust levels where necessary, restricting access to high - dust areas, and ensuring prompt first aid and medical attention for workers showing signs of respiratory distress or eye injury. 6.10 Electrical Injuries: Injury and Illness Management Plan Page 13 of 30 Workers ma y be exposed to electric shock, arc flash, arc blast, burns, and electrocution through contact with energized electrical equipment, temporary electrical
