SALUTE THE GAME | October 13, 2026 1 Participation Waiver & Risk Acknowledgment Salute The Game | October 13, 2026 | Print, sign, and return to Coach B 1. Permission to participate I am the parent or legal guardian of the athlete identified in the registration record and have authority to provide consent. I give permission for the athlete to participate in the Salute The Game basketball tryouts, practices, training, games, camps, and clinics identified in that registration, during the specified season or activity period. This consent does not enroll the athlete in an unselected program or future season. 2. Acknowledgment of activity risks I understand that basketball and physical training involve risks, including falls, contact with other participants or equipment, sprains, fractures, head injuries, heat-related illness, and, in rare cases, serious permanent injury or death. Risks can remain even when reasonable precautions are taken. I understand these risks and voluntarily permit the athlete to participate. 3. Health information and safe participation I will give designated program staff accurate, current information about allergies, medical conditions, injuries, and participation restrictions that may affect the athlete's safety. I will obtain any clearance required by a treating healthcare professional or the program and notify staff of changes. The athlete must follow safety instructions, use required equipment, and stop activity and report pain, injury, or illness promptly. 4. Head injuries and return to play If a concussion is suspected, the athlete must stop participating and tell a coach and parent or guardian. The athlete will not return to play on the same day. Return to sports must follow a healthcare provider's clearance and guidance. This acknowledgment does not replace medical assessment or the program's emergency procedures. 5. Limited release for inherent risks To the extent permitted by applicable law, I release Salute The Game and its program coaches and staff from claims arising solely from the ordinary inherent activity risks described above that I voluntarily accept. This release does not cover negligent conduct, gross negligence, reckless conduct, intentional misconduct, or any claim or right that applicable law does not allow to be waived. It does not purport to waive a minor's legally protected rights. 6. Parent and athlete acknowledgment Before this document is used, the registration must identify the athlete, parent or guardian, selected program or team, season or activity period, document version, and signature date. The parent or guardian must read and sign the approved version; the athlete should review the safety expectations in age-appropriate language. A page view alone is not consent or a signature.
